What is a Life Plan Community?
A Life Plan Community is a senior living community that offers a tiered lifestyle and healthcare options all on one campus. It guarantees access to independent living, assisted living, memory care and skilled nursing and rehab as our residents’ needs change. Residents often enter into independent living and value the peace of mind that comes with knowing additional levels of care are available if their health needs change in the future.
Whatever level of care you receive, you can be assured it will be of the highest quality. We maintain a top rating from the Centers for Medicare & Medicaid Services (CMS), signifying above-average quality at a national level.
What is assisted living?
With the support of a trained, 24-hour staff, you will be empowered to remain independent and connected. Assisted living services include:
- Chef-prepared meals with table service in the dining rooms
- Daily opportunities for learning, creativity, fitness and fun
- Regular housekeeping and linen services
- Emergency pull cords and/or pendants
- Assistance with daily tasks like bathing, grooming and getting dressed
- Medication administration (for an additional fee)
- Incontinence care
Our goal for assisted living is to care for you or your loved one, and that care may not look the same for every person. We believe that care is individualized, and we want to help keep you as independent as possible.
Will I have a roommate in my assisted living apartment?
No. All assisted living apartments are private.
What happens if I run out of money?
One of the biggest concerns of older adults is wondering whether they have enough finances to maintain their standard of living for the rest of their lives. As a way of fulfilling our mission, we make a commitment to residents to provide care for life. Residents can rest assured that they will not have to leave their homes or apartments if their resources are depleted.
Will I need to change doctors when I move into Berkeley Square, Westover or Montage?
No, you may continue seeing the doctors of your choice, and we can even provide transportation to and from your appointments. We do have a medical director who can visit you right in your apartment and pharmacy services that deliver medications directly to you.
Do I have to sign over all of my assets to Berkeley Square, Westover, or Montage?
No. You will only pay for goods or services that you have received.
Can I have a pet?
Pets are allowed in independent living and certain areas of assisted living.
Can I bring my own furnishings?
Yes. Whether you’re a resident in independent or assisted living, we consider our campuses YOUR home and want you to fill your home with what you love.
What if I have a medical emergency?
All homes and apartments are equipped with emergency alert systems and/or pendants.
Is there transportation available?
Yes. Getting to medical appointments or local errands is offered for a fee by The FLEET, Community First’s transportation division. With over 20,000 trips a year, The FLEET provides a vital door-through-door service, making sure that our residents and older adults in Butler County get to their appointments and back into their homes safely.
Are meals available?
Yes, our dining rooms are available all day long for those in assisted living, memory care and skilled nursing. Meals are also available for room delivery when needed.
If I live in independent living, do I have to move when I need assisted living services?
Yes, our independent living apartments and houses are not licensed by the Ohio Department of Health. Assisted living services are delivered in the area of the community licensed as such. Our maintenance team will help make that move an easier transition when/if the time comes.
Does Medicare pay for assisted living or independent living?
No, Medicare is designed to cover short-term in-patient rehabilitation stays. There is not coverage for long-term living accommodations unless you have long-term care insurance, which often covers assisted living and nursing care. *individual policies differ
